Bayside Mobile Physio is set up to provide Hospital in the Home (HITH) physiotherapy across Sydney, delivering the physiotherapy component of HITH and Hospital Substitute Treatment (HST) programs for private hospitals and health insurers, in coordination with the referring hospital or insurer’s clinical team. This service sits alongside our broader Post-Surgical & Post-Hospital Rehabilitation service, for patients continuing recovery once a HITH admission has ended.
Hospital in the Home is a program that allows eligible patients to receive hospital-level care at home instead of remaining as an inpatient, under the direction of a hospital or health insurer’s clinical team, an approach recognised within NSW Health’s rehabilitation model of care. In NSW, HITH is most established within the public hospital system, generally delivered through the hospital’s own employed physiotherapy staff rather than external providers.
Private hospitals and health insurers increasingly offer an equivalent, often structured as a Hospital Substitute Treatment (HST) program, and it’s here that external providers like Bayside Mobile Physio are engaged directly to deliver part of that care in the patient’s home.
We can provide the physiotherapy component of care. Where a private hospital, insurer, or Hospital Substitute Treatment program needs a physiotherapist attending a patient at home as part of their HITH or HST care, Bayside Mobile Physio is available to provide that clinical physiotherapy component. We don’t provide the medical or nursing components of HITH care, such as IV therapy or wound dressing, our role would be the physiotherapy, mobility, and rehabilitation side of the program.
Open to direct engagement from hospitals and insurers. Private hospitals and health insurers running HST or HITH programs often engage mobile physiotherapy providers directly, rather than relying solely on hospital-employed staff. Bayside Mobile Physio is set up to be engaged this way, from the start of a patient’s HITH care, not only after formal discharge.
Coordinated clinical handover. When engaged as part of a HITH admission, we liaise directly with the hospital’s medical and physiotherapy team, so treatment continues on the same plan rather than starting over.
Physiotherapy across the full scope of a HITH admission. Depending on the patient’s condition, that can include gait and mobility retraining, falls risk assessment, and pain management, not just general mobilisation.
Continuing care once a HITH admission ends. Once a patient’s formal HITH admission closes, we can continue physiotherapy with them as they move into standard post-discharge rehabilitation. Older adults in particular often go on to longer-term care through our Aged Care Physiotherapy Sydney or Support at Home Physiotherapy services, so there’s no second handover or gap in care.

Standard post-hospital physiotherapy, like the rest of our Post-Surgical & Post-Hospital Rehabilitation service, is the ongoing rehabilitation that continues once that acute phase or hospital admission has ended.

During an initial consultation, a thorough history will be taken. Your main concerns will be identified and assessed. Your condition will be explained clearly to you and then treated appropriately. Treatment will vary depending on your condition but may consist of:
Manual (hands on) therapy
Massage therapy
Hot/Cold therapy
Exercise therapy
Education
Review any functional difficulties at home or at work with modifications as required
Option 1 – Private clients
Payment is arranged after your appointment. We will contact you to process payment
Option 2 – Support at Home & Aged Care Clients
If your services are funded through Support At Home Program, payment is managed directly with your provider.
Self-funded clients are required to pay within 14 days of receiving their invoice.
Option 3 – For NDIS Clients
Plan-managed: Payment is processed by your plan manager after each session.
Self-managed: You will receive an invoice to pay within 14 days.
Here’s how payment works depending on your funding type:
Private clients — Payment is arranged after your appointment.
Support at Home (aged care) — Payment is managed directly with your aged care provider. No out-of-pocket cost for eligible clients.
NDIS — Plan-managed participants have invoices sent directly to their plan manager. Self-managed participants receive an invoice to process through the NDIS portal.
DVA — A referral from your GP is required prior to your first appointment. Payment is claimed directly through DVA.

No. You will need to pay the full appointment fee after your session. We will provide you with a receipt containing all the relevant details so you can submit your claim to your health fund online or via your local health fund office.
Appointments must be cancelled at least 24 hours or more prior to your appointment time, and also within our listed office hours.
Appointments cancelled less than 24 hours prior to the appointment time, or failure to be present at the treatment site at the time of the scheduled appointment, will be charged the full treatment cost.
Cancellations for NDIS appointments will be charged as per governing bodies and policies.
