Support at Home physiotherapy in Sydney, delivered in the comfort of your own home.
Bayside Mobile Physio provides home visit physiotherapy for older Australians receiving Support at Home funding, including people who previously received a Home Care Package.
We help improve mobility, strength, balance and independence. Approved physiotherapy is a clinical support with no participant contribution, subject to available funding and provider authorisation.
We work with registered aged care providers to coordinate referrals, appointments, clinical reporting and direct provider invoicing.
Referrals welcome from care partners, aged care providers, clients and families. Appointment availability depends on location, clinician capacity and provider approval.
Support at Home is the Australian Government’s in-home aged care program. It replaced the Home Care Packages Program and Short-Term Restorative Care Programme on 1 November 2025. The Commonwealth Home Support Programme (CHSP) is a separate program.
Under Support at Home, eligible older Australians receive an assessed budget to access approved services. Your registered provider coordinates your care and confirms how physiotherapy fits within your support plan.
Ask your care partner about adding physiotherapy to your support plan. You can request Bayside Mobile Physio as your preferred physiotherapy provider.
Send us a referral with the client’s goals, location and approved appointment arrangements. We coordinate scheduling and provide clinical updates as required.

Your registered provider confirms your available budget, the services in your care plan and the price charged under its arrangements. Bayside Mobile Physio invoices the provider directly in accordance with the agreed referral and service arrangements.

We aim to make referrals straightforward for registered providers, care partners and families.
Complete our online physiotherapy referral form with the client’s details, suburb, goals, relevant risks and provider contact information.
We liaise with the registered provider to confirm service approval, funding arrangements, appointment duration and any reporting requirements before commencing treatment.
Once authorised, we contact the client or nominated representative to schedule the initial assessment. We provide the agreed clinical documentation and invoice the provider directly.

Our administration team reviews referrals promptly, checks service coverage and confirms the next steps with your team.
We can provide initial assessment reports, treatment recommendations and progress summaries to support care planning and reviews.
We invoice your organisation in line with the agreed billing arrangements, rather than requiring the client to manage physiotherapy claims.
Our standard Support at Home physiotherapy rates include travel within our serviced Sydney areas, subject to referral acceptance and coverage.

Eligibility, assessments and pathway-specific funding are determined through the relevant Support at Home arrangements.
Support at Home generally supports people aged 65 and over, or 50 and over for Aboriginal and Torres Strait Islander people, who meet the program’s assessment and eligibility requirements. Other eligibility provisions may apply.
Physiotherapy can be arranged when it is appropriate to the participant’s assessed needs, included in the support plan and approved by the registered provider.

Appointments are subject to clinician availability and travel coverage in the client’s suburb.
Physiotherapy is a clinical support under Support at Home. Approved sessions can be funded through the participant’s Support at Home arrangements, subject to their care plan, available funding and provider authorisation. The participant contribution rate for clinical supports is 0%.
You can ask your registered provider to engage Bayside Mobile Physio as your preferred physiotherapy service. The provider must agree to the arrangement and authorise the service before appointments commence. Learn more about self-managed Support at Home physiotherapy.
A GP referral is generally not required to arrange physiotherapy through Support at Home. Your registered provider needs to confirm that physiotherapy is appropriate and approved under your support plan.
We aim to organise appointments promptly after receiving a complete referral and provider authorisation. Timing depends on the client’s location and available physiotherapists.
The physiotherapist reviews the client’s mobility, strength, balance, functional needs and goals, then recommends a treatment plan. Initial assessments are generally scheduled for up to 60 minutes, subject to the provider’s approval.
Please give at least 24 hours’ notice when cancelling an appointment. A late cancellation charge may apply under the agreed service terms, subject to applicable exceptions and provider arrangements.
