Continuing Your Recovery After Discharge

A safer first few weeks home. The period right after surgery or a hospital stay is when patients are most at risk of falls, complications, or losing the mobility gains made in hospital. Starting physiotherapy at home, rather than waiting for a clinic appointment, keeps that momentum going.

Coordinated with your hospital team. We liaise with the hospital discharge team and physiotherapy department to arrange ongoing care for patients being discharged, so recovery continues smoothly from the hospital physiotherapy team into home-based care, without a gap in treatment.

No travel while you’re still recovering. Getting to a clinic appointment shortly after surgery, joint replacement, or a hospital admission is often the hardest part. We come to the patient instead.

One physiotherapist, consistent care. Patients see the same physiotherapist through their recovery, so treatment builds session to session rather than starting over with someone new each visit.

Common Reasons Patients Need Physiotherapy After Discharge

This is particularly relevant for private hospital patients. Public hospitals generally deliver Hospital in the Home (HITH) care through their own hospital-employed physiotherapy staff, with no choice of external provider. Private hospitals and health insurers, on the other hand, often contract mobile providers like us directly as part of a Hospital Substitute Treatment (HST) or Hospital in the Home program.

Private hospital physiotherapy teams and health insurers refer patients to us for a range of reasons, including:

  • Supporting Hospital in the Home (HITH) programs, where a patient is discharged earlier than usual but still needs hands-on clinical rehabilitation and monitoring
  • Deconditioning, loss of strength, and reduced endurance after an extended hospital stay or bed rest
  • Continuing rehabilitation goals set by the hospital physiotherapy team, so progress made as an inpatient isn’t lost in the gap before outpatient care starts
  • Reducing the risk of hospital readmission from falls, complications, or functional decline in the first weeks home
  • A patient not yet safe or able to travel to outpatient clinic appointments
  • Limited transport or no carer available to bring the patient to a clinic
  • Assessing and addressing safety risks in the patient’s actual home environment, stairs, bathrooms, uneven flooring, that a clinic visit can’t identify
  • Bridging the wait for a community or outpatient rehabilitation placement

  4. Your physiotherapist works from hospital discharge notes where available, and liaises with the hospital team if needed.

Bayside Mobile Physio have all the equipment needed to treat our patients comprehensively .

Light, comfortable clothing such as shirt and shorts are fine.

During an initial consultation, a thorough history will be taken. Your main concerns will be identified and assessed.  Your condition will be explained clearly to you and then treated appropriately.  Treatment will vary depending on your condition but may consist of:

  •     Manual (hands on) therapy

  •     Massage therapy

  •     Hot/Cold therapy

  •     Exercise therapy

  •     Education

  •     Review any functional difficulties at home or at work with modifications as required

Option 1 – Private clients

Payment is arranged after your appointment. We will contact you to process payment

Option 2 – Support at Home & Aged Care Clients

If your services are funded through Support At Home Program, payment is managed directly with your provider.

Self-funded clients are required to pay within 14 days of receiving their invoice.

Option 3 – For NDIS Clients

  • Plan-managed: Payment is processed by your plan manager after each session.

  • Self-managed: You will receive an invoice to pay within 14 days.

 Here’s how payment works depending on your funding type:

Private clients — Payment is arranged after your appointment.

Support at Home (aged care) — Payment is managed directly with your aged care provider. No out-of-pocket cost for eligible clients.

NDIS — Plan-managed participants have invoices sent directly to their plan manager. Self-managed participants receive an invoice to process through the NDIS portal.

DVA — A referral from your GP is required prior to your first appointment. Payment is claimed directly through DVA.

No. You will need to pay the full appointment fee after your session. We will provide you with a receipt containing all the relevant details so you can submit your claim to your health fund online or via your local health fund office.

Appointments must be cancelled at least 24 hours or more prior to your appointment time, and also within our listed office hours.

 

Appointments cancelled less than 24 hours prior to the appointment time, or failure to be present at the treatment site at the time of the scheduled appointment, will be charged the full treatment cost.

 

Cancellations for NDIS appointments will be charged as per governing bodies and policies.
