A hospital stay changes things. Your parent comes home a little unsteadier on their feet, a little more hesitant on the stairs, and the recovery seems slower than anyone expected. Most families don’t know there’s a specific funding stream built for exactly this moment.
What Is the Restorative Care Pathway?
The Restorative Care Pathway is part of the Australian Government’s Support at Home program. It replaced the old Short-Term Restorative Care Programme in November 2025. It provides up to 16 weeks of intensive, goal-directed support after a hospital stay, a fall, or a sudden change in health. It sits on top of any regular Support at Home services your parent already receives, with its own dedicated funding, separate from the regular quarterly budget.
Each episode provides around $6,000 in funding. If your parent’s needs call for more intensive support, a Support Plan Review can unlock a further $6,000, bringing a single episode to up to $12,000. Up to two episodes are available in a 12-month period, with a mandatory three-month gap required between them.
Who It’s For
The pathway suits people who want to rebuild function and stay independent at home, rather than move straight into a higher, ongoing level of care. It’s built for short-term, intensive recovery, not long-term support.
It isn’t available if your parent is on the End-of-Life Pathway, already in permanent residential care, receiving transition care, or has had two episodes without the required three-month gap between them.
What’s Included
Physiotherapy is a core part of the pathway, focused on rebuilding strength, balance, and mobility, and reducing fall risk. Depending on the goals set with your care team, the pathway can also include occupational therapy, exercise physiology, dietetics, and nursing. Clinical services like physiotherapy are fully government-funded, with no out-of-pocket cost to your parent.
How It’s Different From the Regular Support at Home Budget
Support at Home already includes an ongoing quarterly budget for services like physiotherapy. The Restorative Care Pathway is different. It’s a separate, time-limited funding stream, built specifically for recovery after a health event, not for ongoing maintenance. Using it doesn’t reduce your parent’s regular budget.
How to Ask for It
This is the part most families miss. The Restorative Care Pathway isn’t always raised automatically. If your parent has recently been in hospital, had a fall, or had a sudden decline in function, ask their My Aged Care assessor or existing provider directly: “Is my parent eligible for the Restorative Care Pathway?” If they already have a care manager, ask them to check eligibility and make the referral before defaulting to a lower level of ongoing support.
