If you or a family member self-manage a Support at Home package, one of the biggest advantages you have is choice –  including the choice of who provides your physiotherapy. Many self-managed clients don’t realise that mobile, in-home physiotherapy can be booked directly using their Support at Home funding, without waiting on a provider’s internal roster or travelling to a clinic.

This guide explains how self-managed Support at Home works, how physiotherapy fits into your funding, and how Bayside Mobile Physio supports self-managed clients across Sydney.

If you’re still used to calling it a Home Care Package, this is the same funding under its new name – Support at Home replaced Home Care Packages in November 2025.

What Is Self-Managed Support at Home?

Support at Home replaced the Home Care Package program in November 2025. Once assessed and approved, you’re given a funding classification that sets your quarterly budget, and you choose a registered Support at Home provider to work with.

From there, you can choose how your package is managed:

  • Fully managed — your provider coordinates services and assigns staff on your behalf.
  • Self-managed — you take an active role in choosing your own workers and directing your care, while your registered provider continues to handle care management, compliance, and safety oversight in the background (typically around 10% of your budget).

Self-management doesn’t mean going it alone. Your provider still supports you — it simply means you decide who delivers your services, rather than having a provider assign someone for you.

Can You Use Your Support at Home Funding for Physiotherapy?

Yes. Physiotherapy is a core allied health service funded under Support at Home, commonly used for:

  • Falls prevention and balance retraining
  • Regaining strength and mobility after a hospital stay
  • Managing chronic pain, arthritis, or general stiffness
  • Ongoing strength and mobility maintenance to support independence at home

Under a self-managed arrangement, you can choose the physiotherapist or physiotherapy provider you want – rather than accepting whoever your provider has on hand.

Why Self-Managed Clients Often Choose Their Own Physiotherapist

A few reasons self-management appeals to people using physiotherapy support:

  • Continuity — the same clinician attends your sessions, rather than a rotating roster of unfamiliar staff.
  • Timely starts — you’re not waiting on a provider’s internal availability to begin treatment.
  • Flexibility — sessions are scheduled around your actual needs, not a fixed slot.
  • More of your budget toward hands-on care — rather than being spread across administrative overheads.

What to Look for in a Self-Managed Physiotherapy Provider

Because you’re choosing the provider yourself, it’s worth checking a few things before you commit:

  • Experience specifically in home-based aged care – falls prevention, post-hospital rehab, chronic pain, and mobility support
  • Appropriate professional indemnity insurance and worker screening compliance
  • A genuinely mobile service, rather than a clinic-based practice offering occasional home visits
  • Ability to complete the onboarding process with your registered Support at Home provider – this step varies between allied health providers, so it’s worth confirming upfront before you commit.

How Bayside Mobile Physio Supports Self-Managed Clients

Bayside Mobile Physio provides home-visit physiotherapy across Greater Sydney, delivered by a network of AHPRA-registered physiotherapists. We work alongside your existing registered Support at Home provider — you nominate Bayside as your chosen physiotherapy provider, and we coordinate directly with your provider on invoicing and documentation, so there’s minimal admin on your end.

Common Situations We Help With

Self-managed clients typically bring us in for:

  • Falls prevention – building lower limb strength and balance confidence at home, particularly after a fall or hospital discharge.
  • Post-hospital recovery – regaining mobility and function following surgery, illness, or an extended stay.
  • Chronic pain and arthritis management – ongoing, hands-on treatment to support day-to-day comfort and movement.
  • Maintaining independence – regular sessions focused on preserving strength and mobility over time, rather than waiting for a decline to address.

Getting Started

There are two simple ways to begin:

  1. Contact your Support at Home provider or plan manager and let them know you’d like to nominate Bayside Mobile Physio as your physiotherapy provider.
  2. Contact us directly, and we’ll liaise with your provider or plan manager on your behalf to get things set up.

Either way, referrals are simple – a quick form or email is all it takes to get started.
