Physiotherapists can assess and prescribe assistive technology under the Assistive Technology and Home Modifications (AT-HM) scheme. Bayside Mobile Physio provides AT-HM prescriptions for mobility aids and equipment as part of our Support at Home physiotherapy services across Sydney, including walking frames, walking sticks, and other mobility-related items.
The Assistive Technology and Home Modifications (AT-HM) scheme is a separate funding stream within the Australian Government’s Support at Home program, alongside a client’s regular quarterly budget. It covers two categories: assistive technology (equipment such as mobility aids) and home modifications (structural changes like ramps or grab rails). Items are funded across three tiers based on cost and complexity, low (under $500), medium (up to $2,000), and high (up to $15,000 or more), with higher-cost items requiring a formal assessment and quote before approval. Full scheme details are available on the Department of Health, Disability and Ageing’s AT-HM scheme page.
Under the AT-HM scheme, a suitably qualified health professional, such as an occupational therapist or physiotherapist, can assess and prescribe assistive technology items. Home modifications are generally arranged separately through a qualified building professional. Bayside Mobile Physio’s registered physiotherapists assess and prescribe the assistive technology side of AT-HM, mobility aids and equipment, rather than structural home modifications.
Mobility Assessments. We assess movement, balance, and functional mobility in the home to identify what equipment is actually needed.
Prescribing Equipment. We recommend and formally prescribe items such as walking frames, walking sticks, and other mobility aids suited to the person and their home environment.
Ongoing Review. We reassess as mobility changes over time, so equipment keeps pace with the person’s needs rather than becoming outdated.
Walking aids (frames, crutches, sticks), mobility and transfer support items, and falls prevention equipment prescribed for home safety.
Contact your Support at Home provider or Bayside Mobile Physio directly to discuss changing mobility needs. Book an assessment with one of our physiotherapists, who’ll evaluate mobility, balance, and the home environment. You’ll receive a prescription and quote, submitted through your Support at Home budget for approval, with higher-cost items going through an additional funding tier approval before they’re confirmed.
Clients in NSW needing broader equipment support outside AT-HM can also look into EnableNSW, the state-run assistive technology program.
AT-HM prescriptions are available to Support at Home participants who need assistive technology to stay safe and independent at home. See our Support at Home Physiotherapy page for full eligibility details, or call My Aged Care on 1800 200 422.
Care managers can refer a Support at Home client for an AT-HM mobility assessment the same way as any other physiotherapy referral. See our For Referrers page for our full referral process.
If you or a loved one needs a mobility aid or equipment assessed and prescribed under AT-HM, contact us and we’ll guide you through it.
