    Who We Accept Referrals From

    Support coordinators, care managers, GPs, hospital discharge planners, and families across Greater Sydney. Our team responds to all referrals within 2 business hours and keeps you updated throughout your client’s care.

    What Happens After You Refer

    Once you submit the referral form, our team will be in contact within 2 business hours to arrange an initial assessment. We will keep you informed throughout the process and provide progress updates as required.

    Funding Types We Work With

    We accept referrals for clients funded through NDIS, Support at Home/Home care package, DVA, Medicare Chronic Disease Management, private health insurance, and self-funded arrangements.

    Yes — family members, carers, and guardians are welcome to make a referral on behalf of their loved one.

    We typically arrange an initial assessment within 48 to 72 hours of receiving your referral.

    Yes — we keep referrers informed throughout the client’s treatment with regular progress updates on request.
